This Lutron Skylark® Single-Pole Fully Variable Fan Control is ideal for ceiling fans.  Slide up to on/increase speed; down to decrease/off.  For use in single paddle fan applications.  5 amps.  100% factory tested.  Many other colors available.   

Wallplate not included.  A screwless wallplate can be purchased separately.  See Lutron # CW-1. 

To be installed by a licensed electrician.

      Yes, the SFS-5E-WH can be used with a screw type wall plate. This dimmer doesn't include any wallplate as mentioned in the long description.
